Online Forms and Filing Fee Information
View and download forms and view filing fees for various courts. Forms for statewide use include Family Court, Domestic Violence, Surrogate's Court, Matrimonial/Divorce, Criminal, Attorney-Client Fee Dispute, DWI Ignition Interlock, Name Change, Employment, Request for Judicial Intervention, Interview of Treating Physician, and Health Insurance. Some local court forms are included.

Pay Buffalo Parking Tickets Online
Pay your City of Buffalo parking fine online. A convenience fee applies.
Plead or Pay Traffic Tickets Online
Pay your traffic ticket, or plead not guilty and schedule a hearing. The Department of Motor Vehicles provides this free online service only for Traffic Violations Bureau tickets, not for tickets that must be answered in a local court. The Traffic Violations Bureau only handles tickets in New York City, Buffalo, Rochester, and parts of Babylon, Brookhaven, Huntington, Islip, and Smithtown in Suffolk County.
